I was involved in an accident last week. Glen Lake EMS transported me to the hospital. I need a copy of the report. How do I get this?

    To obtain your medical record you must have either a Consent for Release of Medical Records form signed by the patient or the patient’s guardian, or a subpoena. The request for medical records must be made in writing and should include the patient’s full name and address, and the date and approximate time of the incident. Patient privacy rules authorize the release of personal healthcare information only under these conditions.  The written request for records must be made to the following address:

I just moved into a new home and my insurance company needs to know the distance between my home and the nearest fire station and fire hydrant. Where will I find this information?

My fire alarm just went off. It is a false alarm. Who shall I call to report this?
    First, be sure it is a false alarm! Make sure you check for smoke or fire. If it is a false alarm, you should contact your alarm company. The alarm company will notify the fire department dispatcher. Fire apparatus will generally continue to respond to your home in a non-emergent fashion to make contact with you if you are there or to check your home if you are away. 
Do I need a permit to burn yard trash?
    Yes. The Michigan Department of Natural Resources issues burn permits for both Empire and Glen Arbor townships.  You may obtain one online at (www.michigan.gov/burnpermit). 
The toll free phone number is 1-866-922-2876 (1-866-922-BURN).  If you have specific questions about open burning, you may call the Traverse City Field Office of the DNR at 1-231-922-5280.  Burn Permits are not issued locally by either the Traverse City DNR office or the fire department. 


 
How do I become a volunteer for Glen Arbor Fire Department?
    You must be at least 18 years of age, have a high school diploma or GED, have a current Michigan driver’s license and good driving record. For more information on joining call (231) 334-3279
I have Firefighter/EMS Training/Certification from another state.  Is it transferable to Michigan?
   

Firefighter certifications are administered by the Michigan Office of Firefighter Training.
